Position:
The RN Admissions Nurse will initiate the initial plan of care with authorization of the attending physician and medical director and complete the admission process according to facility policy.
This includes explaining STR services to patients and family members and ensuring all patient s initial needs are met.
Communicate with the Inter-Disciplinary team members regarding initial plan of care, patient s condition, and other essential information.
Strong organizational and interpersonal skills, along with excellent written and verbal communication required.

Requirements:

  • Nursing Home Setting
  • ADN/BSN
  • Minimally - 1 year of experience as a nurse, preferably in a skilled nursing care setting
  • Prior experience in an Admissions Nurse role at skilled nursing, subacute rehab or other fast paced healthcare facility/unit is a must.
  • Experience with Point Click Care (PCC)
  • Equivalent combination of education and experience which provides the required knowledge, skills and abilities as cited above will be considered.
  • Must presents a polished, professional image!
  • BLS/ACLS
  • Current and valid RN license of Pennsylvania
